Dear Parents,

In an effort to avoid the confusion of the previous year, we have put together this sports participation packet to assist you. This packet has all the information that the White House Athletic Department needs for your child to practice with their respective sports team(s). This entire packet is due before the first scheduled practice for the sport in which your child chooses to participate. If it is not returned complete, your child will not be permitted to practice.

Inside this packet you will find two inserts. These inserts are for the physical examination of your child by a medical doctor clearing him/her for athletic participation. The TMA/TSSAA Medical Evaluation Forms (pages 1 & 2) are the forms you will use if you choose to see your own family doctor. The physical must take place after May 1, 2005 and is valid till the end of the sports season/school year in 2006.

Page 3 of the packet or the back page, is an emergency form. This page offers White House your child’s emergency contact information and consent to treat. This page should be filled out completely with detail.

Page 4 of the packet shows a privacy or disclosure form. This form is a Baptist Hospital form which allows White House's athletic trainer to speak to doctors, physical therapists, EMTs, and coaches about injuries that your child may incur during the school year. If this form is not signed, the athletic trainer cannot talk to anyone about the injury, even you the parent(s). Federal law now requires medical facilities to have such forms so as to abide by the Health Insurance Privacy Portability Act. Therefore, no student athlete will be permitted to practice or participate without this form signed.

Once this entire packet is completed by the doctor and you the parent(s), proofread it one last time making sure each signature is filled in. If any signature is missing, the packet will be returned to you and your child will not be allowed to practice until the packet is fully documented.

The TSSAA By-Laws Section 31 mandate applies to all athletes and cheerleaders. No athlete or cheerleader will be allowed to participate in any interscholastic athletic program activity if these forms have not been filed.
